Many rural patients do not realize they have high blood pressure or have otherwise not achieved control. The MAP Framework offers a pathway toward changing that by focusing on helping healthcare organizations Measure BP accurately, Act rapidly to initiate treatment, and Partner with patients and their communities through initiatives like Self Measured Blood Pressure Programs.  Participants will learn & operationalize practices to overcome diagnostic uncertainty, treatment inertia and patient non-adherence.
