Incorporating PharmD as Team-Based Approach to Primary Rural Care

Because of the desert of specialists and subspecialists noticed in rural areas, it is important to utilize all members of the healthcare team in an interdisciplinary approach to meet patient goals. As a clinical pharmacist in rural Southern Illinois, I've piloted a PharmD clinic to work in collaboration with the primary care providers at SIU Family Medicine to meet patient goals. During this talk, I will give several examples that benefited from this team-based approach.
